Scholarship Application + Entry Form

THIS FORM MUST BE ATTACHED TO EACH LITTLE BLACK DRESS SUBMITTED. ENTRY FEE PER STUDENT IS A $35 CHECK OR MONEY ORDER PAYABLE TO "THE FASHION GROUP FOUNDATION OF HOUSTON, INC". YOU MUST BE ENROLLED IN AN ACCREDITED FASHION PROGRAM IN SPRING OR FALL 2010.

DEADLINE FOR ENTRIES: WEDNESDAY, MARCH 2, 2011 4 P.M. Entries must be delivered or shipped to: NEAL HAMIL AGENCY, 7887 San Felipe, Ste. 110, Houston, TX 77063. Office hours are M-F 9-5 pm. 713.789.1335. Clearly mark all shipped boxes "LITTLE BLACK DRESS" and retain your tracking number. We are not responsible for lost shipments. Questions regarding garment drop-off may be directed to the email and phone above. Students are welcome to enter this contest, as well as apply for other scholarships (with other designs) offered by Career Day Houston.

***IMPORTANT—ATTACH ONE FORM PER GARMENT ENTRY***

  1. Provide a photo of your dress with name, school and contact information along with this entry form in a clear, plastic sheet protector and attach it to the garment with a ribbon
  2. Hang garments on a dry cleaner style hanger in a clear plastic bag. No expensive garment bags or hangers, please.
  3. Fashion show garments must be either women's sizes 4-6.

If your garment is not selected to be one of the Top Ten finalists to be in the fashion show, it will be mailed back to you at the address you provide below or available at Career Day Houston. The garments of the top ten finalists will be returned to you following the Little Black Dress event in April. The LBD scholarships will be awarded after the event.
